It is highly recommended that one plans as much of their funeral as possible before they pass. This includes filling out a Catholic Advance Directive, as well as choosing the readings and music ahead of time. 

Below are the appropriate forms and resources. After they are filled out, give them to a loved one or your power of attorney. St. Aloysius and St. Vincent does not keep or save funeral planning sheets before one's death.
